This policy covers information collected through this website. Information collected once you are a patient of the clinic is handled under Alberta's Health Information Act and our in-clinic privacy practices, which the front desk can give you a copy of.

What this website collects

When you use the appointment form we ask for three things: your name, your phone number, and roughly what you need. That is deliberately the minimum needed for someone to call you back.

We also record, alongside your request, the page you submitted from, the time you submitted it, and any advertising campaign identifiers present in the link you arrived through. This tells us which of our ads are working. It does not identify you to anyone else.

If you tick the optional box, we record that you have consented to receive appointment reminders and offers. That box is unticked by default and leaving it unticked has no effect on your request.

We do not ask for your date of birth, your email address, your health card, or your insurance details on this website. If any of those are needed, the front desk will take them over the phone or in person.

What we do with it

We use it to call you back and arrange your appointment, and to answer the question you asked. If you gave optional consent, we may also contact you about reminders and offers, and you can withdraw that consent at any time by telling us.

We do not sell your information, and we do not share it with anyone for their own marketing.

Health information

The "what do you need" field is free text, so it may contain information about your health. We treat anything you write there as health information and it is never passed to advertising platforms, analytics tools, or any third party.

Third parties involved

Running this website involves a small number of service providers:

• Cloudflare, which hosts and delivers this website. It processes technical data such as your IP address in order to serve pages and block abuse.
• Google, for Google Analytics and Google Ads measurement. Google receives that a request was submitted. It does not receive your name, phone number, or what you wrote.
• Meta, for advertising measurement on Facebook and Instagram, on the same basis. It receives that a request happened, not its contents.
• Google Workspace, which carries the email your request generates to our clinic inbox.

Cookies

This site stores a small amount of data in your browser to remember which advertising campaign brought you here, so that we can tell which ads produce appointments. It contains no personal information and you can clear it at any time through your browser settings. We do not use cookies to build a profile of you or to follow you around other websites.

How long we keep it

Requests that do not become appointments are kept for [RETENTION PERIOD, CONFIRM WITH CLINIC] and then deleted. If you become a patient, your information moves into your dental record and is retained for the period Alberta law requires of dental records.

Your rights

Under Alberta's Personal Information Protection Act you can ask what personal information we hold about you, ask us to correct it, and withdraw your consent to us using it. Contact us and we will respond within the timeframe the Act sets out.
